Coach Clearances and Concussion Training

So, you’ve decided to become a coach?    That is awesome! 

Thanks for volunteering for this critical position in our organization.  There are 5 PA State and EPYSA Soccer mandated processes required from our coaches.  Sorry for the time this takes, but it is for the safety of our children/players.

Note: LYSC Does not maintain files year-over-year since many of these steps must accomplished each year.  Previous years are destroyed or deleted.  You are welcome to keep copies of clearances that are valid for multiple years, but you will need to resubmit them each soccer year.

1. KidSafe: EPYSA Risk Management for Fall 2017/Spring 2018   Must be completed on-line each season (Fall and Spring both).  You can’t be placed on a new team roster until this is done.

2. Pennsylvania State Police clearance and PA Child Abuse clearance:  Must be submitted once for the FALL-SPRING soccer year.  If you have copies that are valid (less than three years old), you may submit them.

3. Possible FBI Background check. Only needs completed if you have not lived in PA for the past 10 years.  This also needs to be submitted once for the FALL-SPRING soccer year.  If you have copies that are valid, you may submit them.

4. Concussion awareness on-line training.   Must be done every FALL-SPRING soccer year and the certificate of completion submitted.  (Not valid beyond that soccer year.)

5. LYSC Concussion Policy.    Must be signed and submitted every FALL-SPRING soccer year.  (Not valid beyond that soccer year.)

DETAILS ON HOW TO ACCOMPLISH THESE STEPS:

1. KidSafe: EPYSA Risk Management for Fall 2017/Spring 2018 This only takes a few minutes to complete. Make sure you select the correct club (LYSC) and the correct volunteer position, (coach and what gender/age.)  You can’t be placed on a team as the coach by your “Age Group Coordinator” until this step is completed. Don’t forget, this needs to be done each season Fall and Spring!

2. Pennsylvania State Police clearance and PA Child Abuse clearance:  Fill out THIS FORM, for the Warwick Regional Rec Commission who will submit it to the state agencies on your behalf. Note that the form indicates that there is a $10.00 fee, you can ignore that.  After the state processes these requests for information, YOU will receive the clearances.  You then need to forward them as you did the initial paperwork (DOCUMENT SUBMISSION below.)

3. Possible FBI Background/fingerprint check.  Have you lived in Pennsylvania for the last ten (10) consecutive years?   If you answered ‘yes’, no FBI check is required in this case, but be prepared to provide proof of residency to the LYSC board if requested.  If you answered “No”, you are required to have an FBI background check through the PA department of public welfare. To begin that process, please register through the Cogentid Website. You will need to provide fingerprints for verification. Fingerprinting locations are posted on the Cogentid website and your results will be mailed to you a few days after you provide your fingerprints.  Note: when you provide the reason to be fingerprinted, be sure to mark yourself as ‘Employment with a significant likelihood of regular contact with children.’  There is a cost to register, and the registration fee is fully reimbursed by the club. Email your receipt to treasurer@lysc.org.  After the check it done YOU will receive the results.  You then need to forward them as you did the initial paperwork (DOCUMENT SUBMISSION below.)

4. Concussion awareness on-line training. This is a on-line course brought to you by the CDC.  It takes about 45 minutes, and you will print/save a certificate with your name on it upon successful completion.

5. LYSC Concussion Policy. You will need to sign a copy of this document.  This is the same document your parents will be signing.

Document Submission:  Submit the following documents, (Email or hardcopies).

(Step #2) PA Child Abuse History Application (or valid copies of the Police and Child Abuse clearances.)
(Step #3) Receipt for payment for FBI fingerprints (or results of fingerprint checks) – only if needed.
(Step #4) Concussion Training course certificate
(Step #5) LYSC Concussion policy

EMAIL Method:  Start an email to certs@lysc.org.  In the body of the email include your race and maiden name (if different from your current surname). Attach the required documents and send.

MAIL/DROPOFF Method:
If unable to scan and email, the documents may be mailed to or dropped off at:

We recognize that some of these reports will take several weeks to get back from the state or FBI. Please don’t forget to forward them (email or mail) when you receive them.

Note that if all information needed to file your applications is not received by the deadline of first scheduled game, or the certs are not received within a reasonable amount of time after they have been submitted, you may be removed from the roster and not be permitted to coach until the appropriate documentation is received.

At your first practice:
Distribute these two documents to parents, or send them the paperwork ahead of time. You may find it helpful to put them front-and-back on the same sheet of paper.

You need a completed and signed copy of each of these documents for each player on your team. There are no exceptions: if you do not receive one on behalf of a player, that player cannot be permitted to participate.  When you receive all of them back, keep them in your med kit so that you have them in case you need them.
